Overview:
We are seeking a dedicated and experienced Wellness Director to join our team. As the Wellness Director, you will be responsible for overseeing and managing the wellness programs and services provided to our clients. This is a leadership role that requires strong knowledge of home health, physiology, hospice care, and case management. Responsibilities:
- Develop and implement comprehensive wellness programs for our clients
- Supervise and manage a team of wellness professionals
- Conduct assessments to determine clients' needs and develop personalized care plans
- Monitor patients' progress and make adjustments to care plans as needed
- Provide education and support to clients and their families regarding wellness strategies
- Collaborate with other healthcare professionals to ensure coordinated care
- Maintain accurate documentation of patient records and progress reports
- Stay up-to-date with industry trends and best practices in wellness care
- Ensure compliance with all regulatory requirements, including Medicare guidelines
- Adhere to medical terminology standards in all communications

Requirements:
- Bachelor's degree in a related field (e.g., healthcare administration, nursing, public health)
- Proven experience in a similar role, preferably in a home health or hospice care setting
- Strong knowledge of physiology, medical terminology, and patient monitoring techniques
- Excellent leadership skills with the ability to effectively supervise a team
- Exceptional organizational and time management abilities
- Excellent communication and interpersonal skills
- Ability to work collaboratively with other healthcare professionals
- Proficient in using computer systems for documentation purposes

Job Type: Full-time

Pay: $60,000.00 - $70,000.00 per year
